Purpose: To serve students through a tiered approach with disabilities who are currently in high school and have an Individual Education Plan (IEP) through an individualized approach to assist with developing the necessary tools to progress from high school to adult life.

Philosophy: The high school transition program empowers young adults to achieve their goals and their potential through developing their skills and resources to maximize their independence in their adult life.

Tier 1: identify student needs through interest based surveys and assessments, connect with post secondary counselors, support for college and career exploration

Tier 2: identify student needs through interest based surveys and assessments, connect with post secondary counselors, support for college and career exploration, possible connections with outside agencies, specific transition class focusing on support for accessing resources and supporting individual student goals for post-secondary
